Reporting

Keep Safe Accessibility Issues are being resolved

Pan Software and UNE are working on resolving the accessibility issues within Keep Safe. During this time people with different accessibility requirements can use the WHS F021 Incident Form (DOCX, 43.9 KB) to report hazards and incidents. Please send the completed form to your supervisor and copying in whs@une.edu.au.

Keep Safe

UNE uses a WHS Management System called Keep Safe to report, investigate and track actions required to prevent recurrence or reduce risk associated with a hazard.

Keep Safe uses Single Sign On (SSO) which is accessed once logged on to the Staff intranet site (staff) and myUNE (students). Staff and students can then log in to Keep Safe using the following link:

The University values privacy and is committed to protecting your personal information. The purpose of collecting this information is to aid in the response to, and prevention of injuries and illnesses, as well as to identify and mitigate hazards. Information you provide will be available to your supervisor/line manager and P&C WHS Team. In this way Keep Safe ensures that your information is kept confidential.

In some instances – specialist expertise is required to review and assist with an investigation or remediation of an unsafe situation. In those instances those parties may be requested, in consultation with the worker – to be added to view the incident record (for example, a chemical spill may need specialist advice to contain and remediate the hazard).

It is important that information provided in the report is framed succinctly and appropriately to enable a quality investigation of the circumstances.

Staff can also receive assistance from their Health and Safety Representative (HSR). More information about the role of an HSR can be found here. HSR’s are elected representatives for specific work groups. The HSR’s for each work group are located on the Safety Hub.

Anonymous reporting can be made through Grapevine, however investigation and management of an unsafe situation may be limited by the inability to discuss and fully investigate the matter.
