Unit information
Chronic conditions are the leading cause of illness, disability and death in Australia. Nursing care for people experiencing these conditions is of critical importance. Studying this unit, you will use real-life case scenarios to explore chronic conditions across the lifespan.
Extending your knowledge, we continue to focus on key concepts for safe and effective nursing practice, including applied anatomy and physiology, pathophysiology, diagnostics and pharmacology.
Strengthen your abilities in clinical reasoning, critical thinking, communication and reflective practice, supported by the ethical and legal principles central to holistic nursing care.
You will critically examine end of life care and the nurse’s role in dealing with death and dying, health facilitation and meeting the needs of Aboriginal and Torres Strait Islander people living with complex conditions, as well as chronic disease policy priorities (e.g. Australia’s National Strategic Framework).

Learning outcomes
Upon completion of this unit, students will be able to:
- identify and critically review the nursing assessment and management of common chronic conditions related to health priorities in Australia;
- examine the principles of chronic pain and the pharmacological management of chronic diseases;
- critically evaluate the nurse's role when dealing with death and dying and end of life care;
- discuss the concept of self-management, stepped care and programs and initiatives to support people with chronic illness or disability;
- examine the National Strategic Framework for chronic conditions; and
- identify the nurse's role in meeting the health needs of Aboriginal and Torres Strait Islander people, in particular those people living with disability or chronic conditions.
