COVID-19 Declaration - BIOL110 - University of New England (UNE)

To ensure the ongoing safety of staff and students as they attend campus UNE has developed a COVID-19 Safe plan in conjunction with key stakeholders.

This form needs to be completed and submitted no more than 3 days before commencement of your on campus activity. Successful completion of this form will result in you being awarded a COVID-19 Safe Face to Face Activities Badge within Moodle.

The COVID-19 Safe Face to Face Activities Badge is only valid for Trimester 1 2021.

COVID-19 Health and Safety Procedures

Personal Protective Equipment *

I agree to follow the direction of UNE staff as to what Personal Protective Equipment (PPE) needs to be used whilst on campus eg. masks, safety glasses, gloves.

Cleanliness *

I agree to follow correct procedure in relation to the cleaning of surfaces used in classes and the need for clean hands before and after class participation.

Mobile Phones *

I am aware that it is recommended that mobile phones are not used during class due to the contamination risk to others.

Social Distancing *

I am aware that it is expected that students will wear PPE and/or abide by social distancing standards whilst on campus at UNE (as per NSW Health directives).

Removal from UNE Campus *

I acknowledge that UNE staff have the authority to remove students who do not comply with the above from campus by UNE security staff in accordance with UNE's WHS policies.

Return to UNE Campus Induction *

I declare that I have completed my return to campus induction and received the return to campus induction certificate.
