Unit information
Pharmacists play a critical role in delivering direct healthcare services across diverse and evolving practice settings. This unit provides an opportunity to apply your pharmacy practice skills, knowledge, and professional judgment in real-world scenarios, with a particular emphasis on dispensing, extemporaneous compounding, and medication management services.
Through this unit, you will develop the skills necessary to support safe and effective medication use, focusing on more complex disease states commonly encountered in pharmacy practice, such as oncology and palliative care. You will refine your counselling techniques, ensuring you can confidently educate patients on managing complex conditions, polypharmacy, and medication adherence strategies.
With a strong emphasis on medication management services and medication reconciliation, you will learn how to assess medication regimens, identify potential medication-related issues, and provide evidence-based recommendations to optimise patient outcomes. Special attention will be given to pharmacists' expanding role in areas such as aged care, where they play a key role in safe prescribing, deprescribing, and medication management.
Additionally, this unit will enhance your understanding of the ethical and legal frameworks of pharmacy practice, equipping you to provide culturally appropriate, patient-centered care. You will also explore the evolving responsibilities of pharmacists in specialised clinical settings, recognising the profession’s expanding scope in complex disease management.
Through hands-on activities, case-based learning, and interactive discussions, you will strengthen your ability to provide high-quality pharmaceutical care, ensuring safe, effective, and personalised medication management across various healthcare settings.

Learning outcomes
Upon completion of this unit, students will be able to:
- demonstrate competency in dispensing and extemporaneous compounding by accurately preparing, labeling, and documenting pharmaceutical products in accordance with professional standards and regulatory requirements;
- apply pharmaceutical calculations with precision to support safe medication dispensing, compounding, and patient care in both community and hospital pharmacy settings;
- develop and implement medication management strategies to optimize therapy for complex conditions such as oncology and palliative care, addressing issues such as polypharmacy, medication reconciliation, and deprescribing;
- utilise advanced communication and counseling techniques to educate patients on medication adherence, complex therapies, and medication-related challenges;
- critically evaluate the ethical, legal, and professional responsibilities of pharmacists in evolving healthcare settings, ensuring culturally appropriate, patient-centered care; and
- analyse and apply evidence-based approaches to pharmacist-led services in specialised settings, such as aged care and complex disease management.
