Approval for the requested leave of absence must be obtained from your Principal Supervisor prior to lodging this form.

If you are an international student, you must provide:

A medical certificate or supporting documentation if requesting a leave of absence based on medical grounds.
A statement of support from your Principal Supervisor for all other leave of absence requests.

Candidates experiencing difficulties with the online form below must complete the Leave of Absence form as a Word document and attach it as supporting documentation to the online application.
