CONFIDENTIALITY AND PRIVACY

The UNE Special Needs Office requires health professional documentation from students registering with the Office to assess the impact of the student's situation, disability or health condition on their university study. This enables the Office to provide reasonable study adjustments, based on each student's individual needs. The Special Needs Office understands the often sensitive nature of the information that may be disclosed and respects the privacy of that information. The University of New England is subject to the Privacy and Personal Information Protection Act 1998 and the Health Records and Information Privacy Act 2002.

The personal information you provide in relation to your registration will be protected in accordance with the University Privacy Management Rule and will be used by the University to assist you to meet the inherent requirements of your course with support and practical assistance related to your condition. If you do not complete all the questions on this form and submit relevant documentation, it may not be possible to process your registration. Your personal information will be stored securely. You may access and correct your personal information by contacting disabilities@une.edu.au. The University of New England will not disclose your personal information without your consent unless the University is under a legal obligation to do so.
