INJURY/ILLNESS:
An incident that has resulted in personal harm and/or a health complication

DANGEROUS OCCURRENCE:
Damage or malfunction of any plant, equipment, machinery, boiler or pressure vessel including piping that could have resulted in death, serious injury or substantial damage to property.
