Incident Reporting
Staff Members
It is the employees responsibility to immediately (or as soon as possible after the injury/illness has occurred) report full details of any work related injury or illness to the relevant Supervisor or Head of School, Directorate or College following the application of appropriate first aid and/or medical treatment.
Forms can be obtained from the Occupational Health and Safety Officer and are also available online at 04.05a.doc (Following the submission of the online form by either the employee or relevant Supervisor or Head of department/ School/ Directorate/ College it must be printed and annotated by both parties and sent to the Occupational Health and Safety Officer, Human Resource Services within 24 hours).
A significant injury is defined by legislation as a workplace injury that is likely to result in a person being either totally or partially incapacitated for a continuous period of more than 7 days. In the case of a significant injury a verbal report must be made to the Occupational Health and Safety Officer, Human Resource Services, within 24 hours of the injury/illness occurring.
Students
In the event that a student has a reportable accident under the supervision of an academic or general staff member the same procedure as for staff applies (above).
In the case of accidents occuring within the University / College grounds but not within the academic context, the incident should be reported to the Occupational Health and Safety Officer directly or by using the online form (04.05b.doc)
Visitors
Visitors to the University can report accidents to the Occupational Health and Safety Officer directly or by using the online form 04.05b.doc
