Inter library loan/document delivery registration form
This service is only available to academics and postgraduate students. Undergraduates must have specific permission from their supervisor or course coordinator. Completion of this form will register you to request books and journal articles from non-UNE library collections.
This form is required by the Copyright Act 1968 S49(1) and must be signed and lodged with the Document Services Unit by hand, mail or fax (61 2 6773 3273) before you can use the Interlibrary Loan Request Form.
Privacy - The information you provide here will be used to process your registration and be retained by the Dixson Library for Library purposes only. The provision of this information by you is voluntary. However, if you do not supply all the requested information we may not be able to process your registration and ILL/DD requests. You have the right to access and/or correct any personal information concerning you held by the University, subject to the reasonable convenience of the University.
