EO Adviser Reporting Form

Please submit your form/s by the end of the first week of the month following the period covered (eg end of the first week in March for the period January/February). If you have a nil return please send an email advising this.

These statistics are used to monitor and evaluate University policy and procedures.

Please indicate the number of hours  involved in each issue handled during the reporting period (including preparation, preliminary interviews etc)

If you handled an issue not listed, please specify the issue and indicate the number of hours taken.

Mediation
